Please arrive 15 minutes before your first appointment to fill out all health care forms.
Your first appointment will be 1 hour with the Chiropractor for a full assessment and paperwork with treatment.

- OCF-1 Form filled out (you can download from our forms page or fill out at the clinic)
- Claim number, Insurance Company,
- Adjuster Name, Phone number, and Fax Number
- OCF - 5 Form
- Any Extended Health Care Benefits information
